Creates "Medina County" license plates available to vehicle owners in Ohio, designed by the Medina County Historical Society and approved by the registrar.

Requires a $15 contribution per initial and renewal registration for the plates, deposited into the license plate contribution fund for distribution to the Medina County Historical Society.

Imposes a $10 administrative fee per registration to compensate the Bureau of Motor Vehicles for services, deposited into the public safety-highway purposes fund.

Amends Section 4501.21 to add section 4503.764 to the list of specialized license plate programs and their designated fund recipients, including detailed distribution instructions for numerous charitable organizations and institutions.

Plates must display standard county identification stickers as required by law and can be combined with special reserved license plate requests.
