Requires every employer to provide written or electronic earnings and deductions statements to employees for each pay period on regular paydays.

Statements must include employee name and address, employer name, gross and net wages, itemized deductions with purposes, payment date and pay period, and for hourly employees: total hours worked, hourly rate, and hours exceeding 40 per workweek.

Employees who do not receive required statements may submit written requests; employers must provide statements within 10 days of the request.

If employer fails to provide statement within the 10-day period, employee may report violation to the Ohio Director of Commerce, who may issue written notice requiring employer to post notice at workplace for 10 days.
