Creates "ALS Awareness" license plates in Ohio allowing vehicle owners to register for these special plates with a $25 contribution per registration and renewal, plus a $10 administrative fee.

Contributions from ALS Awareness plate sales are deposited into the license plate contribution fund and distributed to the ALS Association Central & Southern Ohio Chapter and ALS Association Northern Ohio Chapter to fund ALS research, treatment discovery, and support services.

Amends section 4501.21 of the Revised Code to add section 4503.725 establishing the new license plate program and specifying that the plates display ALS Awareness logos and words approved by both ALS Association chapters and the registrar.

The ALS Awareness plates display county identification stickers as required by law and can be combined with special reserved license plate requests under existing statutes.

All investment earnings from the license plate contribution fund are credited back to the fund, with annual distributions to beneficiary entities proportionate to the amounts they received in the prior calendar year.
