Requires health insuring corporations to cap annual copayments, cost sharing, and deductibles at 40% of the total annual cost of providing all covered health care services to a standard population.

Exempts high deductible health plans linked to health savings accounts and catastrophic-only plans from the 40% cost-sharing cap requirement.

Mandates that when calculating an enrollee's cost-sharing contributions, insurers must include all amounts paid by the enrollee and on behalf of the enrollee, to the extent allowable under federal law.

Prohibits health insuring corporations from imposing lifetime maximums on basic health care services, though they may establish benefit limits for inpatient hospital services under supplemental health care agreements.

Applies to health benefit plans delivered, issued for delivery, modified, or renewed 90 days after the effective date of the act.
