Requires all firearm transfers between unlicensed individuals to be processed through a federally licensed dealer or law enforcement agency, with limited exceptions for temporary transfers, hunting, law enforcement use, and transfers to museums or gunsmiths.

Mandates background checks on unlicensed transferees including compliance with federal 18 U.S.C. 922(t) requirements and state incompetency records checks conducted by the attorney general.

Establishes penalties for illegal firearm transfers: misdemeanor of the fourth degree for first offense, second degree for second offense, and first degree for three or more offenses, with mandatory fines applicable to each level.

Allows federally licensed firearms dealers and law enforcement agencies to assess fees up to $10 per firearm transfer to cover processing costs.

Modifies section 5122.311 to authorize attorney general background check records to be used by federally licensed firearms dealers and law enforcement agencies conducting incompetency checks pursuant to this section.
