• Prohibits mandatory vaccinations for all individuals except as specifically provided in sections 3313.671 and 5104.014 of the Revised Code, which govern school and child day-care immunization requirements.

• Establishes state policy protecting individuals' rights to direct their own health care decisions free from coercion or penalty, with informed consent for themselves, their children, and family members.

• Prohibits vaccine status disclosures, vaccine passport systems, and vaccine registries by employers, health care providers, insurers, schools, nursing homes, and government entities, with limited exceptions for patient care, treatment, and billing.

• Bars discrimination against individuals and businesses based on vaccination status or refusal to disclose vaccine information in employment, services, housing, and public accommodations.

• Prohibits public officials from issuing orders during emergencies that violate vaccination choice and privacy rights, and provides individuals remedies through civil action, Chapter 4112 complaints, and attorney general enforcement.
