Prohibits proprietors and managers of places of public accommodation from denying service, segregating, or providing different treatment to individuals who refuse vaccination for any reason

Prohibits state agencies, political subdivisions, child day-care centers, and schools from denying access to public buildings or providing different services to unvaccinated individuals, with exceptions for sections 3313.671 and 5104.014 of the Revised Code

Violations subject to fines of not less than $5,000 per violation, with each instance of negligent treatment considered a separate violation

Creates COVID-19 vaccine fairness fund in state treasury to receive collected fines, split equally between the Department of Development (for assistance to businesses closed by COVID-19 pandemic) and Department of Health (for assistance to individuals injured by COVID-19 vaccines)

Permits health care practitioners and hospitals to ask patients about vaccination status for purposes of medical treatment
