Amends section 5119.61 of the Revised Code to require the Ohio Department of Mental Health and Addiction Services to collect and compile comprehensive data on alcohol and drug addiction services and recovery supports.

Requires collection of admission data including number of new admissions, transfers, patient demographics (age, sex, race, ethnicity, employment, veteran status, pregnancy status), psychiatric diagnoses, arrests in prior 30 days, and substance use history.

Requires collection of discharge data including frequency of substance use during treatment, living arrangements, employment status, arrests in prior 30 days, length of stay, and reason for discharge.

Mandates collection of treatment outcome data including completion rates and relapse rates for persons with alcohol use disorder, substance use disorder, and persons at risk of substance use disorder.

Requires the department to submit quarterly and annual reports to the governor, general assembly, and alcohol and drug addiction boards summarizing and analyzing the collected data, and to post reports on its website.
