Amends Ohio Revised Code sections 173.50 and 173.501 to expand the Program of All-Inclusive Care for the Elderly (PACE), a Medicaid component for frail older adults.

Authorizes the Department of Aging to issue requests for proposals to establish six new PACE organizations serving Franklin, Hamilton, Montgomery, Lorain, Lucas, and Summit counties within 90 days of the bill's effective date.

Establishes eligibility criteria for PACE organizations, including nonprofit or for-profit status, valid provider agreements, feasibility studies, suitable facilities, and compliance with federal requirements under 42 C.F.R. Part 460.

Creates a "home first" component allowing individuals admitted to nursing facilities or at imminent risk of admission (within 30 days) to enroll in PACE services instead, with priority enrollment determined by department rules.

Appropriates $50,000,000 from the Dedicated Purpose Fund in fiscal year 2022 (with unspent amounts reappropriated to fiscal year 2023) to fund expansion of PACE to six total organizations statewide.
