Requires educator license applications for initial licensure or renewal under Ohio Revised Code sections 3319.22 to 3319.31 to include the school district or school where the applicant is employed.

Mandates the Ohio Department of Education implement an annual system requiring all current educator license holders to submit notification of their district or school of employment and report any employment changes.

Requires the Department of Education to notify a school district or school within ninety days when one of its employees becomes subject to a consent agreement or disciplinary action by the Department or State Board of Education.
