Applies to Congress to call an Article V Convention of the States limited to proposing amendments that impose fiscal restraints on the federal government, limit federal power and jurisdiction, and limit terms of office for federal officials and Members of Congress

Establishes reservations including that Congress's only power is to call the convention and set a reasonable time and place, while state legislatures retain exclusive power to name delegates

Requires applications from two-thirds of state legislatures on the same subject before Congress must perform its duty to call the convention

Specifies the convention shall be limited to the stated topics, operate on one-state-one-vote basis, and cannot consider any amendment affecting the Bill of Rights

Directs the Ohio House Clerk to transmit copies of the application to federal officials and legislative leaders in all states, requesting their cooperation
