Requires Ohio schools participating in interscholastic athletics to designate separate teams based on sex: female teams in female divisions, male teams in male divisions, and co-ed teams where applicable.

Prohibits schools and athletic organizations from allowing males to participate on teams designated exclusively for females.

Requires disputed sex determinations to be established by physician's statement based on reproductive anatomy, testosterone levels, or genetic makeup.

Applies same requirements to state institutions of higher education and private colleges that are members of NCAA, NAIA, or NJCAA.

Creates private causes of action for athletes harmed by violations, allowing recovery of damages including psychological/emotional/physical harm, attorney's fees, and costs; establishes two-year statute of limitations for civil claims.
