• Creates the "On Our Sleeves" specialty license plate to support Nationwide children's hospital's mental health campaign.

• License plate applicants pay a $20 contribution to the license plate contribution fund and a $10 administrative fee to the highway purposes fund.

• Contributions from the "On Our Sleeves" license plates are directed to Nationwide children's hospital for the "On Our Sleeves" campaign.

• Amends section 4501.21 to establish how the license plate contribution fund distributes money to numerous charitable organizations, health foundations, schools, and nonprofits across Ohio.

• Investment earnings from the license plate contribution fund are distributed annually to eligible entities proportionate to the contributions they received in the previous calendar year.
