Creates the "Ohio Bird Sanctuary" specialty license plate allowing vehicle owners to register vehicles with plates displaying an appropriate logo and words selected by the Ohio Bird Sanctuary in Mansfield, Ohio.

Establishes a $15 contribution per registration and renewal, plus a $10 administrative fee; contributions are deposited into the license plate contribution fund created under section 4501.21.

Directs all contributions from the Ohio Bird Sanctuary license plate to the Ohio Bird Sanctuary located in Mansfield, Ohio for purposes of its operations, bird care and rehabilitation, and educational programs.

Amends section 4501.21 of the Revised Code to specify that the registrar shall pay contributions received from the new license plate to the Ohio Bird Sanctuary and establishes procedures for collecting and distributing these funds.

Repeals existing section 4501.21 and replaces it with the newly enacted section 4503.569 governing the registration and issuance of the Ohio Bird Sanctuary license plates.
