• Prohibits tanning facility operators from providing sun lamp tanning services to any individual under 18 years of age.

• Requires operators to obtain parental/legal guardian consent on board-approved forms for minors 16-17 years old, limiting such minors to 45 sessions per 90-day consent period.

• Requires operators to obtain parental/legal guardian consent before each session for minors under 16 and mandates parent/guardian presence during each tanning session for this age group.

• Allows physicians licensed to practice medicine and surgery to use titles "Doctor of Medicine" and osteopathic physicians to use "Doctor of Osteopathy" in addition to existing permitted titles.

• Authorizes electronic signatures for tanning consent forms and establishes $500 minimum fines for violations of minor tanning restrictions.
