Enacts section 3701.147 of the Revised Code to establish education and awareness requirements regarding cognitive impairment, Alzheimer's disease, and other types of dementia.

Requires the Ohio Department of Health, in coordination with the Department of Aging, Commission on Minority Health, and community organizations focused on dementia awareness, to incorporate awareness information into public health outreach.

Mandates educational materials address early warning signs, risk factor reduction, importance of early detection and timely diagnosis, and assessment tools for detection and diagnosis.

Requires materials to specifically address cognitive decline risk reduction among individuals from diverse cultural, ethnic, racial, geographic, and socioeconomic backgrounds who are disproportionately affected by dementia.

Mandates inclusion of information on the importance of annual Medicare wellness visits or annual physical exams for individuals age 65 and older, including Medicare billing codes for cognitive impairment treatment.
