Permits students to fulfill high school physical education requirements through participation in club sports or other athletic activities taught by an instructor outside of school.

Defines "club sports" as school-sanctioned sports not regulated by interscholastic conference organizations and limited to participants under 19 years of age, or youth sports organizations as defined in state law.

Defines "other athletic activity" as a sport or physical activity taught by an instructor outside of school.

Requires students participating in club sports or other athletic activities to submit a signed statement to their school district or charter school governing authority, including signatures from the coach or instructor and the student's parent or guardian.

Amends section 3313.603 of the Revised Code and repeals the existing version of that section.
