Requires insurance issuers that offer Medigap (Medicare supplement) policies to individuals 65 and older to also offer the same coverage to Medicare-eligible individuals under 65 who qualify due to disability or end-stage renal disease

Prohibits premiums for under-65 enrollees from exceeding the premiums charged to individuals 65 and older for the same coverage

Bars exclusions or limitations on benefits for preexisting conditions for policies issued to under-65 Medicare beneficiaries

Establishes a six-month open enrollment period (January 1 to July 1) following the law's effective date for eligible individuals under 65 to obtain coverage

Allows existing policyholders under this program to switch to a different issuer offering the same coverage within 60 days of their birthday each year
