Applies to all health benefit plans amended, issued, or renewed after the law's effective date, overriding any existing deductibles, copayments, or coinsurance requirements

Covers devices whether or not a prescription is required to obtain them

Permits insurers to set cost-sharing amounts lower than the statutory caps
