Requires insurance policies, public employee benefit plans, and Medicaid to cover annual screening mammography (including digital breast tomosynthesis), supplemental breast cancer screening for individuals with dense breast tissue or increased cancer risk, and diagnostic breast examinations for those with abnormalities detected through screening

Eliminates all cost-sharing requirements (copayments, deductibles, coinsurance) for breast cancer screening mammography, supplemental screening, diagnostic examinations, and cervical cancer cytologic screening

Changes eligibility language from "women" to "individuals" throughout the relevant code sections, expanding coverage to all adult individuals regardless of gender

Establishes reimbursement floor protections requiring insurers to pay providers at least the same rate previously paid for these services, preventing rate reductions after the law takes effect

Requires the Director of Health and Superintendent of Insurance to notify hospitals, physicians, and health plan issuers of the law's enactment within three months via certified mail
