Establishes a mandatory lead service line replacement program requiring all lead service lines in Ohio to be replaced within 15 years of the act's effective date

Requires public water system owners/operators to submit lead service line replacement plans to the Ohio EPA, including inventories of all lead, galvanized, non-lead, and unknown status service lines with locations and replacement schedules

Creates protections for low and moderate income customers, including payment exemptions, financial contribution requirements from water systems, and prohibition on disconnecting water service for inability to pay replacement costs

Authorizes various local government entities (townships, villages, municipal corporations, counties, conservancy districts, sanitary districts, regional water/sewer districts) to replace lead and non-lead service lines and recover costs through customer rates

Mandates prevailing wage requirements for lead service line replacement projects and requires water systems to provide customers with water filters and educational resources before, during, and after replacement work
