State Medical Board authorized to issue limited licenses to physicians educated and licensed in countries other than the United States or Canada who have a medical degree from a school in the World Directory of Medical Schools, valid ECFMG certification, and have passed USMLE steps 1 and 2

Limited licenses valid for two years with one renewal permitted; applicants must have full-time employment offer from a participating health care entity (hospital, federally qualified health center, or community health center) and pay a $305 application fee

Participating health care entities must assess and evaluate the physician's practice across six competency domains, employ licensed physician evaluators, carry malpractice insurance covering the limited license holder, and submit fitness recommendations to the board

Limited license holders may convert to a full unrestricted license under section 4731.14 after passing USMLE step 3, maintaining full-time employment, and successfully completing all employer assessments with a $305 conversion fee
