Requires health benefit plans and Ohio Medicaid to cover diagnosis and treatment of menopause, perimenopause, and related symptoms, including clinical laboratory services, diagnostic services, and provider visits

Mandates coverage for FDA-approved hormone therapy (oral, transdermal, vaginal, or injection) and nonhormonal treatments for menopausal and perimenopausal symptoms

Allows coverage of non-FDA-approved formulations when FDA-approved treatments are contraindicated or ineffective, without requiring patients to first try FDA-approved options

Prohibits insurers from imposing cost-sharing or utilization management requirements (including prior authorization) more restrictive than those applied to other medical benefits

Bars health plans and Medicaid from classifying menopause or perimenopause diagnosis and treatment as elective or cosmetic
