Defines biomarker testing broadly to include single-analyte tests, multiplex panel tests, protein expression analysis, and whole exome, genome, and transcriptome sequencing

Insurers must provide coverage in a manner that limits care disruptions, including avoiding the need for multiple biopsies or biospecimen samples

Coverage explicitly excludes biomarker testing used solely for general screening purposes
