Mandates that a parent or legal guardian be physically present at the tanning facility for the entire duration of each tanning session for 16-17 year olds

Imposes a mandatory $500 fine for first-time violations of the age verification and consent requirements under Section 4713.50

Amends Ohio Revised Code sections 4713.50, 4713.51, and 4713.64 to update consent form requirements and disciplinary procedures enforced by the State Cosmetology and Barber Board
