Physicians whose medical licenses were revoked may apply to the State Medical Board for reinstatement after 5 years, with a non-refundable $610 application fee

Revocations for mental/physical illness or substance abuse may apply immediately; revocations for felonies are only eligible if the offense was non-violent and unrelated to patient care

Applicants must complete 25 hours of continuing medical education per year of revocation, participate in community service, undergo a professional skills evaluation, and comply with all original revocation terms

The board must find by clear and convincing evidence that the applicant has good moral character, can practice safely, and reinstatement serves the public interest; a criminal background check is required

Reinstated physicians face mandatory one-year probation and must pass a competency examination, with the board authorized to impose additional conditions including ongoing mental/physical evaluations
