Imposes a 5-year waiting period before a new operator can enter into a Medicaid provider agreement for a nursing facility following a change of operator, with exceptions for non-fiscal emergencies

Modifies private room incentive payments for nursing facilities, maintaining rates of $30/day for category one private rooms (unshared bathroom) and $20/day for category two (shared bathroom), with a $160 million annual spending cap

Requires nursing facilities to maintain at least a 2-star CMS quality rating and not be on the Special Focus Facility list to retain eligibility for private room incentive payments

Reduces nursing facility per Medicaid day payment rates by $71 million in fiscal year 2027 and another $71 million in fiscal year 2028 through adjustments to the cost per case-mix unit calculations

Increases the quality incentive payment pool from $125 million to $366 million and modifies the threshold for receiving quality payments, setting it at a fixed 32 points rather than the 25th percentile of all facilities
