SB 1357 Summary

• Directs the State Department of Health to develop a universal transfer form for secure transmission of health information when patients transfer between health care facilities.

• The form must include patient identification, diagnoses, medications, transfer facility names, transfer date, patient condition, reason for transfer, allergies, medical history, and advance directives.

• Requires the completed form to be provided to the receiving facility by or before patient arrival, and a copy to the patient's primary care physician within three business days if applicable.

• Department must publish the form on its website by January 1, 2012, in both print format and electronic Continuity of Care Document (CCD) format for health records systems.

• Authorizes the State Board of Health to promulgate rules for implementation and compliance with state and federal medical privacy laws; defines "health care facility" to include hospitals, surgical centers, long-term care, and other health service providers; effective November 1, 2010.
