Creates the Statistical Abortion Reporting Act requiring physicians to submit detailed Individual Abortion Forms to the State Department of Health for each abortion performed, including patient demographics, gestational age, method used, reasons for seeking abortion, and compliance with informed consent requirements.

Requires physicians to submit Complications of Induced Abortion Reports within 60 days of encountering any illness or injury related to an induced abortion, documenting the type of complication and follow-up care provided.

Mandates the Department publish an Annual Abortion Report by June 1 each year (starting 2013) compiling statistics from all submitted forms, while prohibiting inclusion of any identifying patient information.

Establishes penalties including $500 late fees per 30-day period for overdue reports, potential license renewal denial for reports more than one year overdue, and misdemeanor charges for knowing failure to submit or submitting false information.

Repeals existing abortion reporting statutes (Sections 1-738a through 1-738h) and declares an emergency for immediate effect upon passage.
