Amends Section 5 of the 21st Century Quality Jobs Incentive Act to clarify eligibility conditions for quarterly incentive payments to establishments.

Establishes that an establishment becomes ineligible if it fails to meet wage and job creation requirements within 12 quarters of application approval.

Allows an establishment to lose eligibility if it fails to meet requirements in 4 consecutive quarters during the 28 subsequent quarters after initially qualifying.

Changes the ineligibility trigger from failing requirements in any single quarter during the 39 subsequent quarters to failing in 4 consecutive quarters during a 28-quarter period.

Effective date: November 1, 2011.
