Prohibits qualified health plans offered through an Exchange established in Oklahoma from including elective abortion coverage, effective November 1, 2011.

Prohibits health plans offered outside an Exchange within the state from providing elective abortion coverage, except through optional separate supplemental coverage requiring a separate premium payment.

Defines "elective abortion" as any abortion other than one to prevent the death of the pregnant woman, and prohibits considering potential self-harm as justification.

Requires health insurers offering elective abortion coverage to calculate premiums based on actuarial cost, obtain separate signatures for enrollment, provide notice of optional coverage, and prohibits premium discounts for other coverage based on elective abortion coverage enrollment.

Requires employers and group health plan entities to provide employees and members the option to choose or reject separate supplemental elective abortion coverage at employment start and annually thereafter.
