Requires the Oklahoma Tax Commission and motor license agents to offer vehicle owners the option to make a voluntary $1 contribution to the Oklahoma Sports Eye Safety Program when obtaining or renewing vehicle titles and registrations.

Creates the Oklahoma Sports Eye Safety Program Revolving Fund administered by the State Department of Health, funded by voluntary contributions from vehicle title and registration transactions.

Authorizes the State Department of Health to use fund revenues to explore nonprofit partnerships for providing sports eye safety information and equipment to children under 18, and to establish a grant program for purchasing and distributing sports eye safety materials and protective eyewear to schools and youth.

Amends sections 1105, 1107, and 1115 of Title 47 of the Oklahoma Statutes to add the inquiry and contribution mechanism to existing title issuance, title transfer, and registration renewal processes.

Becomes effective November 1, 2011.
