Amends Title 63 O.S. 2011 to require the State Department of Health to establish, operate and maintain a public umbilical cord blood bank in collaboration with a private blood bank organization by January 1, 2013.

Increases the birth certificate fee by Five Dollars ($5.00) beginning January 1, 2013, with proceeds deposited into a newly created Public Umbilical Cord Blood Bank Fund; this increase is removed after five years.

Requires physicians and hospitals in Oklahoma to provide pregnant patients with information about donating umbilical cord blood and placental tissue to the public bank at no cost, with exceptions for medical necessity and religious objections.

Mandates the State Commissioner of Health request information from private blood banks meeting specific criteria (prior Oklahoma operations, multi-city presence, accreditation, cell collection history) regarding establishment of cord blood collection operations.

Creates a revolving fund designated as the "Public Umbilical Cord Blood Bank Fund" for the State Department of Health to budget and expend monies for establishing and operating the public umbilical cord blood bank; effective November 1, 2012.
