Adds definition of "exclusive provider benefit plan" to insurance law, allowing insurers to exclude benefits for services from non-preferred providers except emergency care.

Modifies the Health Care Freedom of Choice Act to include exclusive provider benefit plans alongside preferred provider organizations in various regulatory provisions.

Extends consumer protections regarding deductibles, copayments, and cost-sharing to apply to exclusive provider benefit plans with same limitations as PPOs.

Amends the Health Insurance High Risk Pool Act and Hospital and Medical Services Utilization Review Act to recognize exclusive provider benefit plans as regulated entities.

Becomes effective November 1, 2012.
