Establishes the "Oklahoma Truth in Adoption Act" allowing adult adopted persons to obtain uncertified copies of their original birth certificates effective July 1, 2013.

Requires biological parents to file contact preference forms with updated medical and social histories, selecting options for desired contact (willing to be contacted, intermediary contact only, or no contact).

Directs the Registrar of Vital Statistics to match contact preference forms with original birth certificates and make social and medical histories available to adopted persons.

Amends 63 O.S. 2011, Section 1-323 regarding confidentiality of vital records while maintaining protections for record integrity and authorizing limited disclosures to law enforcement and health agencies.

Effective date: November 1, 2012.
