Creates the Oklahoma State Government Vital Statistics Modernization Act requiring the State Department of Health to provide online access to birth and death certificates through a portal system at vitalrecords.ok.gov.

Requires the portal system to allow individuals to receive real-time updates on the status of their certificate requests, including specific information and estimated delivery dates.

Mandates that individuals pay for certificates at the time of online ordering, and allows the Department to charge a convenience fee not to exceed 50% of the vendor's fee as of January 1, 2012, subject to State Governmental Technology Applications Review Board approval.

Requires the Department to prominently display on its website information about how individuals can request certificates through the online portal system.

Effective date is November 1, 2012.
