Amends Oklahoma election law to establish specific requirements for voter identification, requiring documents to show name, photograph, valid expiration date, and be issued by the United States, Oklahoma, higher education institutions, or federally recognized Indian tribes.

Allows county-issued voter identification cards to serve as proof of identity without meeting photograph and expiration date requirements.

Permits voters unable or unwilling to provide proof of identity to sign an affidavit affirming their identity and cast a provisional ballot, with false statements punishable as a felony.

Effective date of July 1, 2012, with emergency clause allowing immediate implementation upon passage and approval.
