Requires tanning facilities to have a knowledgeable operator present during operating hours and provide sanitized protective eyewear to customers before each use, which must be worn by all users.

Mandates facilities use timers accurate within plus or minus 10% of selected intervals (remotely operated), limit exposure to manufacturer recommendations, and maintain interior temperatures at or below 100 degrees Fahrenheit.

Prohibits persons under 18 years of age from using tanning devices, with proof of age required through government-issued identification containing date of birth and photograph.

Requires all tanning facility users to sign a written statement acknowledging they have read warnings and agree to use protective eyewear provided by the facility.

Exempts phototherapy devices prescribed by physicians or authorized healthcare providers and private residences providing tanning devices without charge; becomes effective November 1, 2012.
