Amends the Oklahoma Medicaid Program Reform Act of 2003 to require the Oklahoma Health Care Authority to create a sliding scale for premium assistance that reduces assistance to employees as their salary increases.

Authorizes the Authority to expand the premium assistance program to include employers with up to 250 employees (for-profit) or 500 employees (not-for-profit) based on available funding, with foster parents exempt from employer size requirements.

Establishes the Health Employee and Economy Improvement Act (HEEIA) Revolving Fund as a continuing fund to pay for premium assistance programs using Medicaid funds and federal financial participation.

Requires the Oklahoma Tax Commission, Employment Security Commission, and Department of Human Services to cooperate in providing secure electronic transmission of applicant income data to verify eligibility using available state records.

Becomes effective November 1, 2012.
