Creates the Oklahoma Sports Eye Safety Program Act, allowing taxpayers to donate from their state income tax refunds starting with tax year 2012 to fund sports eye safety information and protective equipment for children age 18 and under

Establishes the Oklahoma Sports Eye Safety Program Revolving Fund administered by the State Department of Health to provide grants for sports eye safety programs, materials, and protective wear in schools

Allows taxpayers to donate from state income tax refunds starting with tax year 2012 to support music festivals held in the Historic Greenwood District in Tulsa

Creates the Historic Greenwood District Music Festival Revolving Fund administered by the Oklahoma Historical Society to promote and support music festivals in the district

Taxpayers who donate in error may file for a refund within 3 years of the tax return due date; effective date is November 1, 2011
