Amends 26 O.S. 2011, Section 5-111 to require candidates' declaration of candidacy forms to include both the name as it shall appear on the ballot and the candidate's legal name.

Allows candidates to optionally provide telephone number, e-mail address, and website address on declaration of candidacy forms, with such information made available to the public but not required.

Modifies 26 O.S. 2011, Section 6-101 to specify that a candidate's name on the official ballot shall be as designated on the declaration of candidacy, using gender-neutral language.

Prohibits any prefix, suffix, or title from being placed before or after a candidate's name on the official ballot.

Effective date: November 1, 2013.
