Amends 76 O.S. 2011, Section 19 to establish uniform fees for medical record copies: $1.00 for the first page and $0.50 for each subsequent page for patients and personal representatives.

Requires attorneys, insurance companies, and subpoena requests to pay a base fee of $10.00 in addition to per-page charges plus postage or delivery fees.

Establishes $0.30 per page cost for electronically stored and delivered medical records (maximum $200.00 plus delivery) when entire request can be reproduced from electronic health record system and delivered electronically.

Prohibits charging patients who request their own records any fee for searching, retrieving, reviewing, and preparing records; waives postage charges for electronically delivered records.

Sets maximum cost of $5.00 per x-ray, photograph, image, or pathology slide; becomes effective November 1, 2013.
