Amends 26 O.S. 2011, Section 5-111 to modify the Declaration of Candidacy form requirements for candidates running for office in Oklahoma.

Requires candidates who have been convicted of or are named in outstanding warrants for specified felony offenses under Section 5-105a to disclose the offense name, conviction/warrant date, and county and state of conviction/warrant issuance.

Exception provided that disclosure requirements do not apply to offenses for which the candidate has received a pardon.

Permits candidates to voluntarily provide telephone number, email address, and website address on candidacy forms, with such information made available to the public if provided.

Effective date: November 1, 2013.
