Sets uniform per-page cost for medical records at $1.00 for the first page and $0.50 for each subsequent page for patients and their representatives.

Establishes a $10.00 base fee for requests from attorneys, insurance companies, and subpoenas, in addition to per-page charges and postage/delivery fees.

Allows providers to charge $0.30 per page for electronically stored and delivered records when the entire request can be reproduced from an electronic health record system, with a maximum total charge of $200.00 plus delivery fees.

Prohibits charging patients who request their own records any fee for searching, retrieving, reviewing, and preparing those records, and eliminates mailing fees for records provided by facsimile.

Sets maximum cost of $5.00 for each x-ray, photograph, image, or pathology slide; effective November 1, 2013.
