Amends 26 O.S. 2011, Section 4-120.3 to require funeral directors (as defined in 59 O.S. § 396.2) to execute a form notifying county election boards of deaths of county residents who are registered voters.

Changes the authority for funeral directors from "may" to "shall" execute the notification form, making it mandatory rather than optional.

Funeral director's signature on the notification form must be either notarized or witnessed by two persons whose signatures and addresses appear on the form.

County election board secretaries are authorized to cancel voter registration upon receipt of the properly executed and witnessed/notarized form from a funeral director.

Effective date: November 1, 2014.
