Amends 26 O.S. 2011, Section 11-109 regarding ballots for retention of Oklahoma Supreme Court Justices and Court of Criminal Appeals and Court of Appeals judges.

Adds notice at top of ballot stating: "NOTICE TO VOTER: Vote separately on each justice or judge; they are not running against each other."

Requires ballots to display office number, judge name and age, years of service, appointing governor's name, and the retention question with YES/NO options.

Becomes effective January 1, 2015.
