Amends 26 O.S. 2011, Section 5-111 to modify the Declaration of Candidacy form requirements for candidates in Oklahoma elections.

Requires candidates who have been convicted of or have outstanding warrants for offenses specified in Section 5-105a to disclose: the offense name, conviction date or warrant issuance date, and the county and state where convicted or warrant issued.

Exempts candidates who have received a pardon from the requirement to disclose criminal history information.

Allows candidates to voluntarily provide contact information (telephone number, email address, website address) on the declaration form, which must be made available to the public if provided.

Effective date: November 1, 2013.
