Establishes operational requirements including trained operators on-site, sanitized equipment and eyewear provided at no cost, timing devices accurate within 10%, interior temperature not exceeding 100°F, and maximum usage of once per 24 hours.

Requires State Department of Health to issue annual permits for tanning facilities; violations subject to civil penalties of $1,000 for first offense and $5,000 for subsequent violations, with possible suspension or revocation of permits.
