Death certificates must be filed with the State Department of Health within 3 days of death, with personal data obtained from next of kin or best available source.

Personal data section must be entered into the electronic system provided by the State Registrar of Vital Statistics by July 1, 2012, and delivered to the attending physician or medical examiner within 24 hours of death.

Medical certification portion of the death certificate must be entered into the prescribed electronic system by July 1, 2016, and submitted to the State Registrar of Vital Statistics.

Medical certification must be completed and signed within 48 hours of death by the physician in charge of patient care, or by the attending physician if the primary physician is unavailable.

The act becomes effective November 1, 2015.
