All health benefit plans offered, issued, or renewed on or after November 1, 2015 must provide coverage for diagnosis, evaluation, and treatment of lymphedema.

Covered benefits include equipment, supplies, complex decongestive therapy, gradient compression garments, and self-management education and training when medically necessary and provided by licensed occupational/physical therapists, licensed nurses, or other licensed health care professionals with appropriate experience.

Gradient compression garments must require a prescription, be custom-fit for the covered individual, and exclude disposable medical supplies such as over-the-counter compression or elastic stocking products.

Standard plan deductibles, coinsurance, and other limitations that apply to similar covered services also apply to lymphedema coverage; plans are not required to provide separate benefit limitations or maximums for lymphedema treatment.

Act becomes effective November 1, 2015, and is codified as Section 6060.2a of Title 36 of the Oklahoma Statutes.
